I'm not certain of the designer or maker. However the back support is an interesting construction. Similar to the ebay list of Paul McCobb,

I haven't seen this type of construction very often. Does the back rest slide onto the metal rod?

Oops, sorry. After closer inspection of the Paul McCobb, I noticed that the back rest was attached to, not inserted onto the rear posts.
